Social Worker II - Advancing Home Dementia Care
Community Support- Corner Brook, NL
DUTIES: The successful incumbent will provide specialized services for adults and their families living with dementia. This will include clinical assessments, intervention, follow up and evaluation using advanced skills and specialized knowledge in gerontology and dementia care across adult populations. The incumbent will work as a member of an interdisciplinary team working closely with geriatricians, the collaborative care teams, and other service providers across the continuum. The incumbent will use advanced counselling skills for the development of therapeutic relationships and may offer intervention through individual, family, and group interventions. This position will provide regional consultation to internal and external partners as required. The incumbent will also support the evaluation of the social worker role within the provincial home dementia care program.
QUALIFICATIONS: Advanced clinical knowledge and skills in gerontology and dementia care across populations. The incumbent must have demonstrated competency in interdisciplinary collaboration, advocacy, and strong decision-making skills. The incumbent must demonstrate considerable analytical and facilitation skills together with strong written and oral communication and organizational skills. The required knowledge and abilities for this position would normally have been acquired through progressively responsible work in the provision of care to older adults and leadership experience. Graduation with a Master of Social Work is required, supplemented by at least (5) five years of working with older populations with knowledge in dementia and frailty. Knowledge in RAI ID, RAI HC and comprehensive geriatric assessments is considered an asset. Candidates must be eligible for registration with the Newfoundland and Labrador College of Social Workers. A satisfactory record of work performance and attendance is a requirement.
