About Our Opportunity
As a care coordinator, you are responsible for the assessment, assessment and determination of individuals with disabilities under financial eligibility and support need requirements, as well as monitoring and disciplinary planning of individual support plans. '
Primary Accountabilities
You will work with individuals with disabilities across their life span who require varying levels of support and supervision, including those with problematic support needs and challenging behaviour.
You will assess individuals with disabilities to determine their level of support and implement in-home support plans, as well as recommend and maintain effective vocational or day programs on behalf of eligible individuals.
As a care coordinator, you will work in partnership with individuals with intellectual disabilities and their families as appropriate to meet unmet support needs within a continuum of available programs.
Qualifications and Experience
You are a licensed professional in nursing, social work, Occupational Therapy and physiotherapy plus five (5) years of related experience with your applicable professional association. You must be currently registered or eligible for registration.
A valid driver's license and the ability to travel through various work locations is required.
You must have excellent assessment, interviewing and negotiation skills which are essential in dealing with complex client situations. You will be a team-player who displays sound judgment and well developed skills in critical thinking, problem solving and time management.
You have a good understanding of the community supports available and the dynamics of facilitating the development of individual support networks and supportive service delivery. Knowledge of health and social services tenets, long term care and case management principles, risk management, respectful community development approaches, coordination of appropriate services, and special care laws is required.
We will assess the above qualifications and competencies using one or more of the following tools: written examination, standardized tests; oral presentation; interview(s), interviews and reference checks.
Equivalency
An equivalent combination of training, education and experience will be considered:
- A master's degree in nursing, social work, Occupational therapy, or physiotherapy plus three (3) years of related experience.

Working Conditions
You will work with individuals who require varying levels of support, care and supervision, including those with complex support needs and challenging behaviour.
You will be responsible for traveling and completing client assessments in multiple locations including family homes, group homes, or residential facilities, psychiatric hospitals, forensic hospitals, and correctional facilities. Additional Information
This is a Part-time, term opportunity with an anticipated end date of Jan 10, 2025. The appointment status (‘term’, ‘temporary’ or ‘casual’) is dependent on the start date of the successful candidate.

Salary Range: $1,320.32 - $1,607.75 Bi-Weekly
