Job Summary
The RAI Home Care Coordinator is responsible for providing leadership and coordination in the implementation, and ongoing monitoring and support for the client assessment instrument, care planning and clinical documentation within designated community health programs. Work is performed relatively independently in coordinating schedules, developing, organizing and presenting initial and ongoing education for all clinical staff and managers. The coordinator reviews assessment data, ensures data integrity and quality for reporting to CIHI and works with managers to interpret RAI data and prepare reports. The Coordinator integrates research and clinical evidence into practice. He/she will use problem-solving and facilitation skills to communicate and work closely with managers and clinical staff to review outcomes of care and develop strategies to improve the quality and safety of client care. This position will work closely with similar positions in the other Regional Health Authorities in the province.

Occasionally required to lift or move educational materials and nursing supplies up to 25 lbs.
Constantly required to sit at a computer or sit to attend or facilitate meetings. Regularly stands to give presentations and occasionally drives between community nursing sites and buildings.
A higher than normal level of attentiveness is required for the health and safety of others.
Regularly subjected to time pressures and deadlines with an occasional lack of control over the pace of work, primarily when there are new procedures or products that require immediate education of Nurses.

Job Qualifications
Graduation from an approved/recognized post-secondary nursing education program with a Baccalaureate Degree in Nursing.
A Master's Degree in a health-related discipline is preferred.
Minimum of five years (within the last seven) of community health nursing experience in the Community Support Program.
Current practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL)
Canadian Nurses Association Community Health Nursing Certification is preferred.
Ability to develop, implement and evaluate community based programs and contribute to policy development.
Demonstrated knowledge of the broad services that are delivered through the Community Support Program.
Demonstrated progressive leadership, analytical, problem solving, decision-making and research skills.
Demonstrated ability to work collaboratively and with a high degree of independence and initiative.
Demonstrated time management skills and ability to prioritize.
Possess strong oral and written communication skills as demonstrated by delivering presentations, facilitating group exercises and writing reports.
Ability to establish and maintain effective working relationships.
A valid Newfoundland and Labrador Class 5 Driver's License and the use of a private vehicle are conditions of employment.
A satisfactory record of work performance and attendance is required.
