JOB PURPOSE:
Working collaboratively with the Diabetes Education Centre team, the outreach case manager supports family physicians and primary health care teams in the community by providing short term intensive patient review and counselling within private office settings. S(h)e completes a full clinical assessment on referred clients establishing a baseline for intervention as well as ongoing monitoring and evaluation. In collaboration with primary health care providers, the case manager is able to review and organize the chart (including patient history, lab test results and developing monitoring tables), develop effective care plans, provide intensive counselling, and make recommendations for appropriate treatments, referrals or changes to therapy.
The case manager improves adherence with best practice by enhancing clinical management as well as patient knowledge and self management skills. In many cases, this involves recommendations regarding initiation and adjustment of medications, including insulin. The case manager takes leadership in the development of this non-traditional role. (S)he has primary responsibility for marketing the role and successfully embedding it in physician office practices. This highly self-directed position requires excellent interpersonal skills and the ability to communicate effectively with a broad group of stakeholders, as well as demonstrated strong clinical skills in chronic disease management. Flexibility in working with emerging technology in office practice settings, including the use of electronic health records will be required. Extensive travel throughout the designated region will be required. This position involves transporting equipment and supplies to various locations which can involve significant lifting and carrying. The case manager may be required to fulfill duties within the Diabetes Education Centre when required.
QUALIFICATIONS:
  • Minimum of a Bachelor Degree in Nursing
  • Must be registered with and a member in good standing with the Nurses Association of New Brunswick
  • Current Canadian Certified Diabetes Educator certification
  • Current certification in insulin therapy and dose adjustment (preferred) OR required to obtain certification within minimal possible timeframe once in position
  • Must have knowledge and skill in chronic disease management gained through a minimum five (5) years experience working with patients with complex chronic disease, including two (2) years with patients with diabetes.
  • Current Basic Cardiac Life Support (BCLS) certification and annual recertification.
  • Ability to travel extensively throughout the assigned region as required.
  • Valid driver's license
  • Must be capable of lifting and carrying program materials and supplies
  • Demonstrates knowledge and understanding of the clinical management of diabetes, including the potential micro- and macrovascular complications, and its psycho-social impact on patients and their families.

KEY RESPONSIBILITIES:
The Diabetes Outreach Case Manager is responsible to:
  • Assist in the provision of comprehensive care for complex patients with diabetes and its associated comorbidities in the community practice setting.
  • Plan and coordinate the care of complex patients with diabetes.
  • Serve as a resource to physicians, primary health care providers and other community groups regarding best practice in diabetes management.
