This competition is to hire one (1) term full-time position ending April 7, 2023, working 75-hours bi-weekly in Care and Community Branch of Continuing Care.

This posting will be used for anticipatory permanent vacancies during the eligibility period.

Who we are

Continuing Care is committed to the development and delivery of comprehensive care and services, providing long-term care, home care and regional therapy services for citizens of Yukon.

Home care delivers health-related services to individuals who have difficulty accessing services in the community due to mobility or health restraints. The objective is to support individuals so that they can live independently in their homes.

You role

The Care and Community Branch of Continuing Care support multiple programs including the intake and assessment for both home care services and Continuing Care facilities. We are looking for a Registered Nurse that has knowledge of Continuing Care as well as intake and assessment processes. Reporting to the Manager, Community Care, you will be responsible for the intake of referrals for Continuing Care programs, performing assessments, evaluating care requirements, and assigning clients requiring long-term services to subordinate staff.

Essential Qualification:

  • Eligible for registration as Registered Nurse or Nurse Practitioner with Yukon Registered Nurses Association;
  • Experience working as a Registered Nurse in a community setting;
  • Experience in assessing and evaluating care requirements for clients, including use of standardized assessment; and,
  • Experience utilizing electronic client records such as Goldcare, Meditech, Plexia, etc.

Desired Knowledge, Skills and Suitability:

Candidates should have and maybe assessed on:

  • Strong ability to collaborate and work within a multi-disciplinary team and with other health professionals, community and government agencies;
  • Knowledge of intake and assessment tools and procedures;
  • Excellent clinical judgment and decision-making skills when assessing information and needs of clients; and,
  • Knowledge of RAI assessments an asset.

Additional Information:

Conditions of Employment: Vulnerable Sector Security Clearance, TB Screening, CPR Level C, Transportation of Dangerous Goods Certificate, WHMIS 2015, Valid Class 5 Driver’s Licence or equivalent without daylight restrictions, and current registration with the Yukon Registered Nurses Association upon hire.

Job Requirements: Lifting up to 20 lbs,, travel within the Territory, and exposure to pets, second-hand smoke, plants, flowers, and latex products.
