Community Health Registered Nurse II (Care Facilitator/Educator), Temporary Full - Time, Community Nursing, Regional, TBD

Definition of work: This is a leadership nursing position which will work in collaboration with the Home and Community Management Team. This position will report to the Regional Manager of Community Health Nursing supporting all aspects of community health programming and education inclusive of palliative care. The successful candidate will collaborate with other community program leaders such as the home first coordinator, acute home care coordinator, wound care consultants and the palliative care consultants. This position will also provide coverage for the acute home care coordinator where required. The successful candidate will be responsible for leading quality improvement initiatives within the department ensuring that best practice guidelines are met and integrated within the program. This position will help develop and execute program goals and objectives, support the achievement of Central Health's strategic plan and integration of the Home First Framework along with other program mandates. The successful candidate will participate in program related QI teams, complete other committee work, data collection and interpretation, policy development, education and orientation. This position will also partake in case load management reviews, monitoring and mentoring of nursing performance and workload measurement processes, and other related duties as required. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.

Required qualifications:
  • Graduation from an approved/recognized post-secondary nursing education program with an undergraduate degree in Nursing or Registered Nurse with an approved Diploma in Community Health Nursing.
  • Current registration with the Association of Registered Nurses of Newfoundland and Labrador.
  • A minimum of five (5) years clinical experience.
  • Current LEAP education training or willingness to complete within one year.
  • Demonstrated recent experience working in other nursing leadership positions.
  • Minimum three (3) years current experience within the last five (5) years in Community Health Nursing.
  • Current BLS certification for health care providers from the Heart and Stroke Foundation.
  • Certification in Community Health Nursing or willingness to complete within two years.
  • Certification in Palliative Care Nursing or willingness to complete within two years.
  • Demonstrated exceptional leadership qualities and problem-solving skills, as well as oral and written communication skills.
  • Demonstrated ability to work well independently and with others in an interdisciplinary team environment.
  • Demonstrated knowledge of the principles of adult learning and performance review, as well as, a commitment to actively participate in the process.
  • Demonstrated understanding of the evaluation process and the CQI process.
  • Demonstrated commitment to personal continuing education.
  • Demonstrated ability to work with computers including the CRMS and Meditech systems.
  • Demonstrated ability to prepare and present educational material in an effective and concise manner with superior interpersonal skills.
  • Demonstrated ability to develop comprehensive learning needs assessment of clients and staff and work objectives based on results of assessment.
  • Demonstrated knowledge of current nursing research and practice standards.
  • A valid Newfoundland and Labrador class IV driver's license, and the use of a private vehicle are conditions of employment.

  • A satisfactory record of work performance and attendance is required for this position.
  • A satisfactory Certificate of Conduct from the RCMP or local authority.

Hours of work: 75 hours bi-weekly (Shift work and flexible work schedule may be required)
Salary: NS-31 ($36.78-$45.18)
