Bi-Weekly Hours: On Demand / 8 hour shifts/ Days, some evening work may be required.(To Create a Casual List)The purpose of this position is to work within a multi-disciplinary team in a youth mental health day treatment program. The Social Service Worker role will work under the supervisor team lead and will work closely with the multidisciplinary team to assist in the implementation of specific behavior management, social, educational, life skills, and recreation programming in accordance with individual and group care plans established by the Team. The incumbent will promote behavioral change and personal growth in youth who are having difficulty coping with co-occurring mental health and possible substance use issues. The position will work primarily in a day treatment program with extensive life skills, recreational or other related programming focusing on reintegrating youth into their communities. The role i s not a traditional counseling role, but rather a supportive counseling role and provides activity-based therapeutic support role for clients within groups or seen individually as a means of fostering behavioral change relationships. Duties will include: - Provide direct supervision for the youth in their care and must be aware of whereabouts at all times.- Promote a nurturing enthusiastic and motivated approach when caring for youth.- Continuously make assessments of youth through observation of their behavior on shift and though specific incidents.- Continuously make assessments of youth through observation of their behavior on shift and through specific incidents.- Act as a role model and demonstrate alternative ways to respond to situation that demonstrate socially acceptable behavior.- Advocate for youth within the community.- Assist, instruct and supervise youth doing specific life skill tasks around the routines of the day.- Assist with transition of clients returning from community or inpatient mental health facilities. - Schedule and assist program staff in development of social, life skills, recreational programming.- Working with teacher to support youth in educational development goals.- Assist with functional readiness skills toward successful program completion.- Assist teachers and counselors in establishing and maintaining contact with appropriate stakeholders.- Contributes to the ongoing assessment process by obtaining/observing pertinent information related to mental status and presenting problem.- Contributes to team based care planning as required.- Contributes to the ongoing evaluation of treatment outcomes and responses to treatment.- Collaborates with the other members of the team and the client/family to ensure client centered care is provided.- Provides supportive counseling.- Provide a range of activity based interventions.- Provide and assist to plan, organize and deliver group programming.- Liaise and support families though phones calls and meeting to enhance a family’s ability to facilitate behavioral change, problem solved, communication strategies, establish boundaries and support skills development to strengthen family functioning.- Provide crisis intervention to youth who present a gander to themselves and others.- Apply non-violent crisis intervention and behavior modification techniques in a variety of settings.- Observe behavior, gather data, and provide input into the development and implementation of individual behavior plans;- On a given shift, might require spending one-on one- time with a youth who requires a high level of involvement for safety or continuous assessment of needs.- Function an activity-based therapeutic support role for clients within groups or individually to assist in fostering behavioral change relationships.- Partnering with other staff and programs to ensure effective case management for youth clients.- Maintaining and providing case documentation including daily case recordings, written and oral evaluations.- Participate in the development and implementation of programs and services as required; and other duties as required.
Qualifications
Minimum Qualifications: - Proof of completion of a Youth Service Worker Program from a certified college program or a comparable program from a recognized post secondary institution.- Considerable experience working directly with youth with mental health and addiction issues.- Demonstrated ability to apply non-violent crisis intervention, conflict resolution and behavioral modification techniques in a variety of settings. Training and experience working with high-risk youth who may be at risk of harm to themselves or others.- Demonstrated ability to assist other groups/agencies in providing life skill programming which might include anger management, parent information sessions, and substance abuse prevention.- Ability to work independently and as part of a team.- Experience working/facilitating with individuals and groups with co-occurring issues. Knowledge of best practices in addictions treatment and mental health issues.- Knowledge of government systems, program planning and implementation, health system services, collective agreements, public service processes.- Applicant must have CPR, First aid certification and non-violent Crisis intervention training.- DBT & CBT training is an asset- Excellent organizational skills. Class 4 Drivers license.- Strong leadership, interpersonal and collaboration skills.- Excellent problem solving, behavioral management, conflict resolution and facilitation skills.
