This posting is Anticipatory for full time permanent positions working 75 hours bi-weekly in Continuing Care Long term Care (Copper Ridge Place, Thomson Centre, & Whistle Bend Place)

Continuing Care provides person-centred care coordination to all residents. The primary focus of this position is to ensure care is holistic, continuous, culturally sensitive and compassionate, and values resident/family experiences. Person-centered care approaches health care as a partnership between the health care provider and resident, where the resident’s assessed needs, values, priorities, goals guide the care planning, and implementation processes and they are an equal partner in the planning of care. Every action performed and decision made by this position is founded on respect for the residents, for promotion of their dignity, individual freedom, choice and lifestyle, and meaningful, quality living.

Essential Qualifications

Please submit your resume clearly demonstrating how you meet the following qualifications. Please note selection for further consideration will be based solely on the information you provide in your resume.

  • Bachelor of Social Work or Registered Nurse
  • Experience providing case management
  • Experience developing person-centred care plans
  • Experience completing assessments in respective field an asset

Candidates should have and may be assessed on:

  • Strong ability to collaborate and work within a multi-disciplinary team
  • Excellent organizational, interpersonal and conflict management skills
  • Effective oral and written communication skills
  • Strong skills working with various cultural backgrounds
  • Ability to take a reflective approach to your practice.

Additional Information:

Conditions of Employment: TB Screening, Vulnerable Sector RCMP security clearance, valid Class 5 Driver’s Licence, WHMIS 2015 and current registration with a Canadian regulatory body

Job Requirements: Exposure to pets, exposure to second-hand smoke, latex products, flowers and/or plants and other irritants while in client homes or within facilities. Travel within Whitehorse
